These Terms and Conditions (“Terms”) govern your use of our website and services. By accessing Kona Mobile PT, scheduling an appointment, or using any part of our services, you agree to these Terms. If you do not agree, please do not use our website or services.
Kona Mobile PT is a mobile physical therapy and wellness service operating on the west side of the Big Island, Hawaii. Services are provided on a private-pay basis and are not billed through insurance providers.
You must be 18 years or older to book services, or have a legal guardian or representative do so on your behalf.
Kona Mobile PT provides:
Physical therapy evaluation and treatment by a licensed Doctor of Physical Therapy
Wellness and movement-based guidance for strength, balance, and mobility
In-home or mobile services only (we do not operate a clinic)
These services do not replace emergency medical care, medical diagnosis, or physician-directed treatment outside the therapist’s scope of practice.
Services are provided on a cash-pay basis only (no insurance billing). Payment is due at the time of service unless otherwise agreed upon in writing.
We accept cash, credit card, or electronic payment through approved platforms.
All appointments are scheduled in advance and confirmed by the provider.
Please provide at least 24 hours notice to cancel or reschedule an appointment.
Cancellations made with less than 24 hours’ notice may be subject to a 50% cancellation fee.
No-shows will be charged the full cost of the session.
Exceptions may be made in cases of illness, emergency, or unsafe travel conditions.
By contacting us through phone, email, or online forms, you consent to being contacted by the provider. Please review our full [Privacy Policy] for how your information is handled.
We do not share your data with third parties and do not engage in spam or unsolicited marketing.
You agree to:
Provide accurate medical history and relevant health information
Communicate clearly with the therapist during sessions
Follow the home program and safety guidance provided
Notify the therapist of any changes to your condition or medications
Failure to do so may limit your progress or result in termination of care.
The therapist may refuse or discontinue services if safety concerns arise, or if the care requested falls outside the scope of physical therapy.
Results are not guaranteed. Each client’s progress depends on a variety of personal factors.
The therapist is not liable for injury or harm resulting from misuse of information or activities performed outside the recommended care plan.
All content on this website, including text, logos, design, and downloadable materials, is the property of Kona Mobile PT and may not be copied, shared, or used without written permission.
To the fullest extent permitted by law, Kona Mobile PT and its therapist shall not be held liable for:
Indirect or incidental damages
Client injury resulting from non-compliance
Misuse of services outside the agreed scope of care
These Terms are governed by the laws of the State of Hawaii. Any disputes shall be resolved in the appropriate state or federal courts of Hawaii.
We may update these Terms at any time. The latest version will always be posted on this website with the updated date.
